Sutter North Home Health has been serving patients in the Yuba Sutter area with consistent, quaility care since 1997.

According to Press Ganey (The health care industry’s leading independent vendor of satisfaction measurement and improvement services) in 2004 Sutter North Home Health rated above average in patient satisfaction for all disciplines. Our percentage for patients that are either re-hospitalized or seeking additional urgent care is less than the state average.

In 2004 Sutter North Home Health was recognized by Lumetra, a home health monitoring agency, with an award for “Moving the Bar” on pain management.

Services:

Special programs available through Sutter North Home Health include: comprehensive rehabilitation care (including ortho aftercare), oncology care, in-home treatment of infectious diseases, cardiovascular care, respiratory care, intravenous therapy, post-surgical care, palliative care, wound therapy, speech therapy, occupational therapy, and physical therapy.

Patient Qualifications:

Patients must meet these requirements to stay on service.
    Homebound: For Medicare patients, leaving home must be infrequent and difficult enough to require assistance. For all other insurances, please confirm with our office at 530-749-3450.
    Need for skilled care: Patients must be in need of skilled, intermittent care from a nurse, or physical therapist.

How to Refer:

A patient referral can by made by anyone. The referral can come from the physician’s office, or from the local hospital. If you think you or someone you know may qualify and benefit from home health services, please call our office directly and we will assist you.
