How we compare as an agency: According to the Press Ganey Patient Survey, Sutter North Home Health rates above average on patient satisfaction for all disciplines. According to the Center for Medicare & Medicaid Services, CMS, our percentage outcomes for patients that are either re-hospitalized or seeking additional urgent care is less than the state average. In October 2004, Sutter North Home Health was awarded the Lumetra Award, "Moving the Bar," for our significant improvement and outcomes for pain management. 5)
How to refer a patient: Referrals can be made from the provider's directly, or from the hospital. Orders from a hospital must specify Sutter North Home Health if you wish to utilize our services.
